  • We work to help protect the rights of HIV positive individuals and their families.
  • We train those living with HIV/AIDS on how to effectively communicate and educate relevant stakeholders and decision makers on HIV/AIDS issues 

Case Management services are vital to a person living with HIV. We guide those impacted through the process with compassion.

Our is a program designed to go into locations that are considered to be:

1. high-risk
2. high-crime environments
3. high-poverty areas

Health promotion and sustaining health programs:

  • offer education about living and staying healthy while living with HIV
  • discussion and information include eating healthy, resting when necessary
  • taking medication as prescribed
  • keeping medical appointments
  • reducing stress and relaxation techniques

Not only do we provide support for those living with HIV/AIDS. We also committed to preventing the spread of HIV by:

  • Providing information about HIV and HIV prevention to individuals, church groups, community organizations
  • Providing information about contraceptives
  • Provide contraceptives free of charge
  • Linkage to PrEP and Pep
  • Linkage to testing
  • Participating in treatment as prevention 

With adequate support, people living with HIV/AIDS are more likely to be able to respond adequately to the stress of being infected and are less likely to develop serious mental health problems.

What is Case Management?

Case management is a multi-step process to ensure timely access to and coordination of medical and psychosocial services for a person livinwith HIV/AIDS and, in most instances, his or her family/close support system. 

The activities in case management are diverse. In addition to assisting clients to access and maintain specific services, case management activities may include negotiation and advocacy for services, consultation with providers, navigation through the service system, psycho-social support, supportive counseling, and general client education. 

To a person living with HIV/AIDS, case management services are essential.

Process & Goals of Case Management

The goal of case management is to promote and support independence and self-sufficiency. There are specific intended outcomes of HIV/AIDS case management for persons living with HIV/AIDS.

Partnership of Georgia, Inc, has a program designed to reach what some consider to be the “unreachable”. The “Take it to the Streets” program, is a program designed to go into locations that are considered to be high-risk, high-crime, high-poverty areas, that most organization will not go, to reach individuals who traditionally will not seek medical care, or sex education by traditional means.


We utilize a tri-intervention approach 

 

1. BEHAVIORAL 
2. STRUCTURAL
3. BIOMEDICAL INTERVENTIONS


Our staff uses developmentally appropriate health materials to educate individuals about HIV prevent and intervention techniques.

The “Take it to the Streets” program provides linkage to STD/STI and pregnancy testing, medical care, food resources, housing as well as other basic needs resources. The Health Partnership of Georgia, Inc., goal is to be able to expand our reach and provide more resources to those individuals who are less likely to be reached by traditional health promotional means.


We provide training/informational sessions for community organization,
community groups, and churches to educate about HIV in their area, HIV prevention, culture (HIV) sensitivity, working with individuals who are HIV positive, erasing stigma, educating staff about resources available to their HIV positive clients, etc.

HIV has touched many families. If you or a loved one is living with HIV, getting and staying in is critical. Ongoing treatment keeps you healthy and prevents the spread of the virus to others. But you don’t have to go through all this alone.
